Live Virtual Visits
For any of the states we serve, if you prefer a face-to-face virtual visit to discuss your concern, log into our HIPAA secure patient portal and electronic medical records system to create your account. You will sign telehealth consent forms, provide proof of your identity, and be directed to choose your appointment time.
Asynchronous Photo Consults
Residents of Illinois, Montana, Nebraska, Oregon, and Washington may choose a photo consult. Simply create your account at your convenience, day or night. You will sign telehealth consent forms, provide proof of your identity, and fill out an intake questionnaire. You may then upload your photos. Photographs should consist of four views of the area of concern; front facing, left, right, and from a distance with adequate lighting. If our nurse practitioner requires additional information or photos to be submitted, you will be notified through the HIPAA secure patient portal.
